Ticket machines
Whether it's for trains, buses or festivals. Without a human to watch them, they just do not do their job.
I arrived at the tiny station where I board the train on some mornings, put my card in the machine, chose the most common ticket type for that line, and paid. Job done.
The next chap has an OAP card that entitles him to discount. Where's the slot for that? There isn't one.
Next chap wants a ticket that isn't listed. Where's the button for that? There isn't one.
It's the first day on the job for the ticket machine, and there's no human there to guide people who don't know what they're doing.
Robots will be taking over the planet at eating our souls next, you wait and see.

By the sounds of it, yours is one of those push button machines, not the new fangled touch screen jobbies?
FGW have recently installed one of the latter (the really new really fast ones) at the station from which I commute most days and, whilst it means there's no longer an excuse for not having a ticket, it is more convenient.
However, in the four weeks it's been there, I've been unable to use it for at least five of the twenty days. Today there was 'hardware service fault [086] call support'. No, you call support! You're wired in for credit card authorisation and ticket price updates, do it yo' damn self.
On another point you mentioned; these machines have every railcard option you need, clearly marked, which is great (I have a YP). However - and this doesn't just apply to this station, but all ticket machines - where's the sodding 'Same Again' button? When you're cutting it a bit fine for the train and there's 3 of you needing the same ticket to get to London for a gig and one of you pays successfully, why can't the next person just say SAME AGAIN!? Why not just buy three of the same ticket and spread the cost? Because using a Young Person's you are limited to buying one ticket per transaction.
It's bloody stupid. They obviously don't get real people to try these things in real scenarios before the release them.
